Position Description

Regional Registered Nurse (Field-Based) BRONX, NY
$58-$60/hr | Full-Time | Contract
Schedule:
Monday-Friday | 8:30 AM - 5:30 PM | 40 hrs/week

We’re seeking an experienced Registered Nurse to provide compassionate, in-home care across assigned territories. This is a 100% field-based role

What You’ll Do Includes:

Conduct home visits for wound care, medication administration, and health assessments
Educate patients and caregivers on care plans and safety
Document visits accurately in the EHR
Collaborate with care teams to support patient outcomes

Requirements Include:

Active NY State RN license
3+ years of clinical nursing experience (home care preferred)
Comfortable working independently in the field
Wound Care experience
Experience with elderly or frail populations strongly preferred
